In the Korean Zen tradition I took lay precepts in 1980, Dharma teacher precepts in 1984, novice monk precepts in 1985, and Bodhisattva (48) precepts in 1986. In 1988 I returned my monastic precepts and became a Dharma teacher again.
In the Vajrayana I have taken Samaya vows as part of initiation into Deity, Tsa Lung and Dzogchen practices. +

I took the Bodhisattva vows with the 17th Gyalwa Karmapa a few years before my first traditional retreat. Later, just before the retreat, we took the lay precepts with Karmapa. I renewed them a few years later with Mingyur Rinpoche and again with Khenchen Pema Sherab Rinpoche during my second retreat. +

Yes, I first took the bodhisattva vow in 2011 with Chokyi Nyima Rinpoche and have renewed it several times. +

After I completed the recitations of the refuge vows, I asked Lama Dawa to guide me on the next step of the path. In 2006 I met with him and Orgyen Rigdzin. Lama Dawa asked a series of questions and I felt as though I were answering them wrong, but at the end Lama Dawa gave me another set of recitations to complete and that was it. I did feel inadequate for assming a huge responsibility as if the well being of all sentient life was now in my care. Honestly if I can just remember to awaken bodhicitta in the morning and dedicate any merit to all beings in the evening at least I have done some small modicum of good. +

I haven't been able to take formally the lay precepts and bodhisattva vows with my main teacher, but since 2014 I have taken many empowerments with him that integrate into them taking these different levels of vows, including the practice commitments.
Besides that, I took bodhisattva vows online with His Eminence Garchen Rinpoche on April 6, 2021. +
