I took refuge with Chagdud Rinpoche in 1991. +
From Chagdud Rinpoche, in October 1995 I received refuge and bodhichitta vows. +
By technicality, at the end of 2008, as part of a final presentation regarding my semester studying Sanskrit in Mysore, India, I sang the refuge formulation in Sanskrit in front of my classmates and teachers. I first took refuge and bodhicitta vows in April 2009 as part of my first Vajrayāna empowerment, with Jigme Tromge Rinpoche. By fortune, I took them in October 2015 with Dzongsar Jamyang Khyentse Rinpoche. By election, I again took them in August 2018 with Semo Saraswati Rinpoche. There's more to the story of trying to take refuge, but I will spare you the details! +
I have taken refuge at Palpung Sherabling Monastery with Guru Vajradhara Kentin Tai Situpa the 18th of October of 2018. After requesting it on an interview given to all the attendants of a teaching he imparted on that ocassion called "Discover the Buddha Within" +

I took refuge with Dungse Thinley Norbu around 1989 with one other person at Sandra Scales house and was given the name, Urgyen Palbar, "Glorious Radiance of Oddiyana. +
